Confirm the items match your forklift, attachment, site rules, maintenance process, and operator training. Add site-specific checks for ramps, dock work, cold rooms, outdoor yards, hazardous atmospheres, or pedestrian controls when they apply.

What the generated sheet records

The form is designed to create a useful shift record, not just a printed list of reminders. The most important fields are the ones that let a supervisor connect a finding to the exact truck, time, operator, and follow-up decision.

  • Company or site name, so the form is not separated from the workplace.
  • Equipment ID, so repeated defects can be traced to the same truck.
  • Shift and operator, so the record matches the person and time of use.
  • Power source, so electric, propane, and diesel checks do not get mixed together.
  • Defect notes and supervisor review, so failed items have a visible next step.

Match the form to the truck type

A one-page form works best when the general truck checks stay consistent and the power-source section changes. That keeps the routine familiar while still catching the issues that matter for battery, fuel cylinder, or engine-powered equipment.

When to use it

Use the sheet before the truck is placed in service. If a truck is used continuously across shifts, review the form at each shift change. If the operator finds a defect that could affect safe operation, record it, report it, and keep the truck out of use until reviewed under your procedure.

After the inspection

A completed sheet should not disappear into a folder before failed items are reviewed. Separate the routine record from active defects: file clean forms, but route failed items to the supervisor, maintenance process, or removal-from-service workflow your site uses. If the same truck or area fails repeatedly, review the pattern instead of treating each form as a separate event.
